Use this customer wallet declaration template when the denominator should come from the customer. The workbook records total category spending across suppliers, the period covered and the evidence behind the statement. It also gives a customer a clear Unknown response. The resulting completeness count tells an account team how many declarations can be considered for a later wallet review.
Each row holds a customer ID, one agreed category, start and end dates, category spend in USD, response basis, response date, evidence reference and the person recording it. Decide the category and whether figures include tax before collecting answers. The source reference can identify a dated questionnaire or meeting note. Keep the underlying document in your own controlled records rather than pasting private correspondence into the workbook.
Keep the amber inputs separate from the pale blue calculation columns. The worksheet has 20 prefilled detail rows, from 13 through 32. If you need more records, extend every affected formula and summary range together, then check a first, middle and final row. Save a dated working copy before replacing the sample.
C-201 declares $250,000 for January through September 2026 and has all supporting fields. C-202 declares an actual zero and is also complete. C-203 answers Unknown, C-204 has reversed period dates, and C-205 has no recorder. The sample therefore has five responses, two complete declarations and $250,000 of complete declared spend. Unknown and missing are not counted as zero declarations.
Record status checks the required fields and chronological period. Unknown is shown separately. A zero amount is accepted when the customer expressly declared zero, while a blank amount remains incomplete. The summary sums spending only from Complete rows. It is a total of recorded category statements, not a share-of-wallet figure, because this collection instrument does not contain your sales numerator.
The workbook is a collection record, not a survey of the whole market. Customers may interpret category boundaries differently or provide estimates. Resolve differences in the record and preserve the original response. Completeness does not establish truth, statistical confidence or permission to distribute customer information. Use identifiers and only share evidence with people entitled to review it.
