Incidents and near-misses reported per thousand units of activity, per business unit, against the firm's own median.

Example

One unit reports 4.1 incidents per thousand transactions, near the firm's median of 3.8. Another reports 0.2. The second is not twenty times safer.

Where it goes wrong

Raw counts, which make the busiest and most diligent unit look worst.
